Social Care and Further Education Pathways
Social Care and Further Education Pathways is a free, voluntary service, which aims to support young people and adults with Special Education Needs and Disabilities (SEND) to improve their wellbeing and find meaningful employment. We also provide fast track support for SEND students in colleges for an effective transition in to working life.
Improving mental wellbeing
Our team will work with you to help you improve your mental and physical health and your wellbeing. We will connect you to local services and organisations that can help you with everyday living such as employment, housing and financial support, as well as introducing you to groups and courses to support you with your mental health and wellbeing.
Our team of Community Link Workers make sure you have access to the widest possible range of support and services to meet your needs and our team of Employment Advisers will support you to find the right job fit and assess what impact employment will have on your benefits. You can opt out of the service at any time, if you feel it is not right for you.
Person centred approach
Each individual will have a person-centred meeting known as a wellbeing discovery session to create a wellbeing action plan. All individuals requiring employment support will be supported by a dedicated Employment Adviser who will provide support to develop a vocational profile that captures their skills, job interests, ambitions and individual support requirements required from an employer. This is part of a holistic approach that focuses on these 4 key areas:
- Independent living
- Community inclusion
- Being healthy
- Preparing for employment
Through your journey on the programme, you will be supported by the Community Link Worker and/or and Employment Adviser, who will create a personalised approach with you to help you with your wellbeing, gain paid employment and then provide ongoing in-work support, also by looking at what adjustments and assistance you might need from an employer. This will enable your new job role to be successful, to develop you in work and enable you to maintain your health and wellbeing.
How Social Care and Further Education Pathways benefits you?
If you are out of work your Employment Adviser will:
• Get to know you
• Create an employment action plan based on what you want to do
• Prepare your CV/practice for interviews
• If required, support with how to positively share your disability with an employer
• Job search and engage with employers
• Advise on welfare benefits and how these could be affected by going into work
Once you enter employment, your Employment Adviser will engage with your employer to:
• have monthly workplace reviews
• Provide an -in-work action plan
• retain an existing role
• Provide ongoing in-work support
• celebrate achievements
• obtain Access to Work funding
Eligiblility
To be eligible for the programme, you must be:
- Over 18 years old with SEND or social care need or be a carer or family member of someone with these needs
- Living in Barnet, Brent, Ealing, Hammersmith & Fulham, Harrow, Hillingdon or Hounslow
- Unemployed

Assigned a Community Link Worker
Wellbeing Discovery Session
A meeting with your Community Link Worker to discuss your needs and get the right support in place. This can be over the phone, video call or face-to-face.
Develop your Wellbeing Action Plan
Access to wellbeing interventions and IPS employment support
Fortnightly catch up with your Community Link Worker or Employment Advisor
Final review of your journey and future goal setting
